US Healthcare Ethics: Pharmaceutical Marketing and Physician Independence

Research shows that physician prescribing behaviour is measurably influenced by pharmaceutical company interactions -- meals, speaker fees, and promotional samples -- even when physicians believe themselves to be unaffected. The Sunshine Act requires public disclosure of industry payments to physicians, yet the practice continues at scale. As a future physician, how do you think about the ethics of industry relationships, and what policies would you adopt for your own practice?

Likely follow-up · What specific cognitive biases explain why physicians underestimate the influence of industry gifts on their prescribing?

US Healthcare Ethics: Reproductive Rights Post-Dobbs

Since the Supreme Court's 2022 ruling in Dobbs v. Jackson Women's Health Organisation, abortion is now governed state-by-state. Some states have enacted near-total bans; others have enshrined broad access. You are an intern in an emergency department in a state with a near-total ban. A 14-year-old patient arrives with a confirmed ectopic pregnancy — a non-viable, life-threatening condition. The hospital's legal team has instructed physicians to wait for explicit signs of clinical deterioration before intervening, citing ambiguity in the state's medical exception language. The attending tells you this is 'out of your hands.' What are the ethical dimensions of this situation?

Likely follow-up · What is the established medical consensus on the treatment of ectopic pregnancy, and why does it constitute emergency care independent of any abortion law?
