The University of North Texas Health Science Center Texas College of Osteopathic Medicine (UNTHSC TCOM), founded in 1970, is the first osteopathic medical school established in Texas and one of the largest DO programs in the country by class size. Located in Fort Worth, it became a state-supported school under the University of North Texas system in 1975. TCOM trains osteopathic physicians with a strong emphasis on primary care and service to Texas communities, and uses the Texas-specific TMDSAS application system.
What makes it different
TMDSAS application (not AACOMAS) — Texas public schools use TMDSAS, making TCOM administratively distinct from most DO programs. TCOM is the oldest DO school in Texas (founded 1970) and among the largest nationally by class size (~230). State law reserves at least 90% of seats for Texas residents, making it one of the most in-state-focused DO schools. Neither Casper nor AAMC PREview is required.
Course & teaching
Integrated curriculum. 4-year DO curriculum with osteopathic manipulative medicine integrated throughout. Years 1–2 at the Fort Worth campus; Years 3–4 clinical rotations at affiliated hospitals and community sites across Texas. TCOM has a long-established clinical network as one of the oldest and largest DO schools.

Interview
UNTHSC TCOM (DO) interviews via MMI (multiple mini-interview), conducted virtually. UNTHSC TCOM conducts a virtual Multiple Mini Interview (MMI) at its Fort Worth campus. Applicants rotate through timed scenario stations assessing communication, ethical reasoning, and commitment to Texas community health. As a Texas public DO school using TMDSAS, interviewers probe osteopathic philosophy, DO shadowing, and commitment to serving Texas — particularly rural and underserved communities. Strong in-state preference means interviews are predominantly with Texas residents.
