How many US medical schools require CASPer in the 2026–2027 cycle?

30 of the 305 US programs we track require CASPer outright this cycle: 23 of 163 MD programs, 5 of 64 DO programs, and 2 of 78 dental programs. A further 2 MD programs require one situational judgement test but accept either CASPer or AAMC PREview. Participation changes every year, so always confirm on the school's admissions page before you skip (or book) a test date.

CASPer is an online situational judgement test delivered by Acuity Insights (formerly Altus). You watch short video or text scenarios and type open-ended responses under time pressure. Schools receive a quartile score and use it to screen for professionalism, empathy and ethical reasoning.

What is AAMC PREview and which schools use it?

AAMC PREview is the AAMC's own situational judgement exam: 30 multiple-choice scenario sets scored on a 1–9 scale. 14 MD programs require it in the 2026–2027 cycle. It is only used by MD programs — DO and dental schools do not accept it.

Do DO schools require CASPer?

Most DO programs do not currently require CASPer — 5 of the 64 DO programs we track require it this cycle. Several others recommend it or accept optional scores, which is a different thing from a requirement; check the school's AACOMAS listing and admissions page.

Do dental schools require CASPer?

A small number do: 2 of 78 US DMD/DDS programs require CASPer in the 2026–2027 cycle. Dental applicants should also watch for school-specific interviews and situational assessments listed in ADEA AADSAS.
