Tutor insight
Miami's MMI will include scenarios reflecting the school's diverse urban patient population — expect communication scenarios involving patients with limited English proficiency and questions about cross-cultural healthcare delivery. If you speak Spanish, highlight this prominently; it is a genuine asset in the Miami clinical environment. Jackson Memorial Hospital rotations are exceptionally clinically rich but also demanding — demonstrate resilience and adaptability in your responses. Miami's lower median MCAT/GPA relative to peer private schools reflects the school's holistic review emphasis on clinical experience and community engagement alongside academics.

US Healthcare Ethics: Medicare, Overtreatment, and Physician Incentives

Medicare's fee-for-service payment model historically rewarded physicians for volume — more procedures, more revenue. Research has consistently shown that regions of the US with higher Medicare spending per capita do not have better outcomes. A 78-year-old Medicare patient with stage IV lung cancer and declining functional status is asking your opinion about whether to pursue a fourth-line chemotherapy regimen that offers a 5% chance of significant response. How do you counsel her, and what does this scenario reveal about physician incentives in US healthcare?

Likely follow-up · What is the difference between the physician's obligation to inform versus recommend, and does it change at the end of life?

Should the US adopt single-payer healthcare?

The US spends far more per capita on healthcare than any other wealthy nation yet has worse outcomes on several measures and tens of millions uninsured or underinsured. Some advocate a single-payer ('Medicare for All') system, others favor building on the ACA's mixed model. What do you see as the strengths and weaknesses of the current US approach, and how would you reform it?

Likely follow-up · Why does the US spend so much more without proportionally better outcomes?
